Freedom of Information Act (FOIA) Template
The
following artifacts are recommended to be requested from the various
government bodies. Please continue to check for new artifacts.
Each
government body will have its own FOIA request form and contact person.
Make sure to visit the government body website to confirm the correct
FOIA contact and form before proceeding.
The following text can be used in either the description or records sought fields when submitting a FOIA request,
"I am requesting any and all electronic copies of (Insert Artifact Name) for years (X-Y)2009, in electronic format. The
principal purpose of the request is to access and disseminate
information regarding the health, safety and welfare or the legal rights
of the general public, and not principally for personal or commercial
benefit. A fee waiver is requested. If there is a fee for this request please contact me before proceeding."
